Driving Records in Missouri

In Missouri, you can request your driving record or that of other drivers from the Missouri Department of Revenue (DOR) Motor Vehicle and Driver Licensing Division (MVDLD). This record contains your driving history including your driver’s license suspensions, revocations, accidents caused, citations and other information. It is important to order your Missouri driving record. This record will help:

  • Employers to use them as a tool for pre-employment background checks
  • Courts for to use in arbitration proceedings.

Types of Missouri Driving Records

These are different types of Missouri driving records:

  • Driving record without personal information — contains driving history, traffic violations and traffic accidents.
  • Driving record with personal information — contains the name and address of the driver, traffic violations, traffic accidents, driving history, driver’s license number and social security number, recent photograph, phone number and information on disability and medical history.

Missouri Department of Revenue adds points to your driving record when you violate any traffic rules or cause accident. These points can lead to license suspension, car registration suspension, fines and higher insurance rates. Employer, Insurance Agency and Attorney Requests

Under Driver’s Privacy Protection Act, attorney, employer or insurance company may request someone else’s driving record. So in general someone else’s driving record can be requested:

  • By Government agencies to be used in official business
  • By court to use them in arbitration proceedings
  • By insurance companies to use them when determining your insurance premiums
  • By employers to use when conducting pre-employment background check.

Ordering your Driving Record without Personal Information

By Mail

To request your Missouri driving record without personal information through mail, you’ll need:

  • A written request that includes your date of birth, full name and driver’s license number
  • Your mailing address
  • Payment.

In Person

You can visit Missouri Division of Motor Vehicle Office to request your Missouri driving record without personal information. You will be required to provide:

  • Your full name, date of birth and Missouri driver’s license number
  • The driving record request fee.

Requesting your Driving Record with Personal Information

You can also request your Missouri driving record with personal information. You’ll need:

  • To complete Form DOR-4678 (complete a Request for Security Access Code)
  • To mail your request to the address found on the top of the form.

Missouri DOR will approve your request and assign you a security access code. This code is used whenever you are requesting your driving record with your personal information.

Ordering Your Own Missouri Driving Record

You can order your own Missouri driving record by mail, in person or by email or fax.

By Mail

For mail request, you need to:

  • Complete Form DOR-4681 (Request From Record Holder)
  • Provide your driving record fee.

In Person

For in person request you will need to visit your local Missouri DMV. Carry with you:

  • Complete Form DOR-4681 (Request From Record Holder)
  • Check or money order.

By Fax or E-mail

For fax or email request, fax or email the following:

  • Complete Form DOR-4681 (Request From Record Holder)
  • Complete the debit or credit card authorization section on the application form to pay the fee.

Fees for MO Driving Records

The following fees apply:

  • $5.88 for Driving record and $0.50 additional fee if ordered by fax or $2 if ordered in person.
